What to expect:

• Dr. Houtan does not accept any insurance, and payment is due at the time of service.
• New Patient appointments cost $400, and last up to 90 minutes.
• • New patients are required to prepay $400 for their Initial Consult. If the patient fails to attend their appointment or cancels/reschedules within 24 hours, this payment will not be refunded.
  • Patients will not be scheduled until payment for the initial consult has been receieved.
• Follow-up appointments are structured as follows, and are scheduled using the doctor’s discretion and the patient’s need:
• • Standard Follow-Up Consults and Lab Review Consults: $180 for 45 min.
  • Extended Follow-Up Consults: $300 for 75 min (for those patients who require more care).
  • Abbreviated Follow-Up Consults: $80 for 20 min (for those patients who require a quick check-in).

Please note:

• New patients are expected to complete their online paperwork at least 48 hours prior to their scheduled appointment to allow the doctor ample time for review. If paperwork is not complete within this time, the patient will need to be rescheduled and is subject to a $125 fee.
• All appointments, if canceled or rescheduled within 24 hours, are subject to a $125 fee. This fee must be paid in full prior to rescheduling with Dr. Houtan.
• All payments are non-refundable.
